In a February 2022 episode of the Ockham Healthcare podcast, Dr Clare Sieber shared her experience as a former member of the General Practice Committee (GPC), offering insight into how general practice is represented at a national level.

What is the General Practice Committee and why does it matter?

The General Practice Committee plays a central role in representing GPs across the UK, particularly in negotiations relating to contracts, funding, and workload.


Although its work happens at a national level, the outcomes are felt directly within practices and PCNs. Decisions shaped through this process influence how services are delivered, how practices operate, and how pressures are experienced locally.


Representation, negotiation, and differing perspectives

A key challenge of the GPC is the complexity of representing a diverse profession. GPs do not operate with a single, unified perspective, and views vary depending on role, geography, and local pressures.


This creates an inherent challenge within any representative structure: how to balance competing priorities while maintaining a coherent negotiating position.


These same dynamics are often seen within partnerships and PCNs, where differing viewpoints can lead to friction if not managed constructively.


Communication and transparency across the system

Maintaining clear communication between national bodies and local practices is not straightforward.


Where communication is perceived as limited or unclear, it can lead to:

  • Misunderstanding of decisions or direction

  • Reduced trust in leadership structures

  • Increased tension within teams and networks


These patterns are familiar at practice and PCN level, where clarity and shared understanding are essential to effective governance.


The impact on GP Partnerships and PCNs

National uncertainty often translates into local pressure. Differences in how changes are interpreted, in particular around contracts or system direction, can surface within partnerships or across PCNs.


These are rarely purely technical disagreements. More often, they reflect differences in priorities, risk appetite, or values.


Without structured dialogue, these differences can become entrenched, affecting both relationships and decision-making.


Aligning leadership in a changing system

As general practice continues to evolve within Integrated Care Systems, the need for alignment within organisations and across them becomes more important.


One of the biggest challenges facing the GPC is achieving this alignment at scale. At a local level, the same challenge exists in a more immediate and practical form.


Creating space for open, structured conversations is often the difference between productive disagreement and ongoing conflict. Mediation can play a role here, supporting practices and PCNs to work through complex issues while maintaining working relationships.
